WHO IS ELIGIBLE:
Drawing is open to all U.S. Citizens who are 18 years or older and legal residents of the United States except Puerto Rico and other US territories. Arize Officers, Employees, Directors, Supervisory Committee Members, and their family members are not eligible to enter. All federal, state and local laws and regulations apply. Void where prohibited or restricted.
HOW TO ENTER:
Automatic entries: Use your Arize issued Mastercard® credit card or debit card on 07/16/24 and/or 07/17/24 for purchases on Amazon.com. Each qualifying transaction counts as one eligible entry. Qualifying transactions are purchase transactions on Amazon.com with transaction dates of 07/16/24 and/or 07/17/24 transacted by an Arize member in good standing. Balance transfers, fees, cash advances, returns, and ATM transactions do not qualify. Alternate entry method: Other eligible individuals interested in participating in the drawing may enter by submitting a written request with your name, email, address, city, state, ZIP code, and telephone number in type or print to Arize, c/o the Marketing Department, 650 Science Park Rd, State College, PA 16803 via U.S. Mail. One (1) mailed-in entry per person allowed. All U.S. Postal entries must be postmarked between 07/16/24 and 07/17/24 11:59:59 PM eastern standard time and received at the Credit Union by 07/17/24, 11:59:59 PM eastern standard time. Arize is not responsible for late, lost, illegible, incomplete, misdirected, misrouted, or inaccurate entries and such entries will be voided.
DRAWING, PRIZE & PRIZE REDEMPTION:
Arize, whose decision is final, will randomly select one (1) winning entry from all eligible entries to receive one (1) $100 Amazon.com gift card. The winner will be notified on or around 07/26/24 by email or telephone. If the winner cannot be contacted or does not respond in 14 days, the winner will be disqualified and a substitute winner will be randomly selected from the remaining entries.
Odds of winning depend on the total number of eligible entries received during the promotion period. The prize is provided by Arize No substitution or transfer of prizes will be allowed. Arize reserves the right to substitute a prize of equal or greater value. All prizes are awarded “as-is” and without warranty of any kind. The winner will be required to sign and return the AFFIDAVIT OF ELIGIBILITY and LIABILITY/PUBLICITY RELEASE, within 14 days from the date printed on the affidavit, to accept the prize. If the winner does not execute and return the AFFIDAVIT OF ELIGIBILITY and LIABILITY/PUBLICITY RELEASE within the specified time period, or is not a member in good standing, then an alternate winner will be selected without further notice from Arize. Any prize notifications returned to Arize as undeliverable will be forfeited and an alternate winner will be selected. By accepting the prize, the winner gives Arize permission to use his/her name and/or photograph in Arize publications without compensation. Prize acceptance also constitutes that the winner is solely responsible for any federal, state and local taxes associated with the prize.
